Agent of Record Letter
LWCC closely follows the state statute that governs how policyholders can request to change their Agent of Record (also known as Broker of Record – BOR). Below are the important steps to follow to make this process easy for everyone involved.
Please make sure you have the correct person sign the Agent of Record (AOR) request. Per state law, only the owner of the policy is authorized to sign an AOR, or in the event that the first-named insured is an entity, only covered individuals listed as being members or officers of the entity are authorized to sign. Signatures from any other person could result in LWCC not honoring the AOR as submitted.
Key information needed includes:
- Policy number
- Effective date of change (can be in future, but cannot be in the past)
- New Agent (Include Agency Code)
- Current Agent
- Name of policy owner (covered member/officer)
- Signature of policy owner (covered member/officer)
- Date of signature
Please submit the Agent of record request via one of the following documents:
- LWCC Agent of Record Letter Form
- The policyholder’s letterhead (Not agent’s letterhead)
- ACORD 36
Email all AORs to agentofrecord@lwcc.com. This address is monitored at all times and is the best way to ensure the document is reviewed promptly for consideration.
